I have read the Parent Information Packet and understand the expectations and requirements for both myself and my child. By enrolling my child, we agree to follow and uphold these guidelines. I understand that failure to meet these expectations or any misconduct may result in suspension of participation or removal from the team.

I grant permission for my child to be photographed and/or filmed during activities, events, and practices. I understand these images or videos may be used by Paradise Cheer for promotional, marketing, social media, and program-related purposes.

As legal guardian of the child listed above, I hereby consent for him/her to participate in tumbling classes/practices held by Paradise Cheer. I recognize that any activity involving height or motion can create the possibility of injury including permanent paralysis and even death from landing or falling on the head or neck. I hereby forever release the Paradise Cheer officers, directors, agents, and employees from all liability for any and all damange and injuries suffered or contracted as a result of participation in any activity at Paradise Cheer.

I understand the payments and fees associated with participation in the Paradise Cheer Full Year Program. I agree to make all payments by their due dates and understand that failure to do so may result in my child’s removal from the team.

By registering you agree to pay a non-refundable $300 registration fee+ $1,365.00 in program fees. Please select how you would like to pay:
